Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.
RN responsibilities may include:
- Skilled nursing visits in the patient’s home
- Start of care, recertification, resumption of care, and discharge assessments
- OASIS documentation and care planning
- Medication reconciliation and patient education
- Wound care and chronic disease management
- Communication with physicians, families, therapists, and the care team
- Vital sign monitoring
- IV Infusions
- Support for chronic disease management, including advanced heart failure, transplants, etc
- Documentation of patient progress and changes in condition
- Helping patients remain safe, independent, and supported at home

Requirements
- Active Virginia RN license
- Current CPR certification
- Reliable transportation
- Valid driver’s license and auto insurance
- Home health experience preferred
- OASIS experience preferred for RN applicants
