About the Role

We are seeking a full-time Physical Therapist (PT) to join our home health team. In this role, you will provide one-on-one care in the home setting, helping patients regain independence, improve mobility, and safely perform daily activities.

You will be responsible for evaluating patients, developing individualized plans of care, and overseeing therapy services, while maintaining a high level of independence in the field. This role is ideal for a clinician who is motivated, compassionate, and thrives in a patient-centered environment.

Key Responsibilities

  • Perform initial evaluations, reassessments, and OASIS documentation

  • Develop and manage individualized treatment plans

  • Provide skilled therapy interventions to improve mobility, strength, balance, and function

  • Supervise and collaborate with Physical Therapist Assistants (PTAs) as applicable

  • Monitor patient progress and adjust plans of care as needed

  • Educate patients and caregivers on home exercise programs, safety, and use of assistive devices

  • Maintain accurate and timely documentation of patient care and progress

  • Participate in care coordination with physicians and interdisciplinary team members

Qualifications

  • Active Physical Therapist (PT) license in Virginia

  • Minimum 1 year of clinical experience (home health experience preferred)

  • Strong knowledge of OASIS documentation and Medicare guidelines (preferred)

  • Excellent communication and interpersonal skills

  • Ability to work independently and manage time effectively

  • Positive, encouraging attitude with a passion for patient care

  • Basic proficiency with electronic documentation systems

  • Valid driver’s license and reliable transportation
